OSMPC is led by directors with experience in primary care, rural generalism and hospital medicine, occupational medicine, and health‑system quality improvement — a breadth of expertise that shapes our practical, people‑focused approach.

Our work focuses on the practical application of evidence-based care. In long-term care settings, OSMPC has reduced avoidable emergency transfers by integrating interdisciplinary care teams and overall proactive care planning. We are promoting the use of community paramedicine to manage urgent concerns on-site. We have supported non-drug interventions to address behavioural symptoms in dementia. These initiatives are all based on current research and adapted to fit the realities of frontline healthcare delivery.
